CMS Announces Grantees for Accountable Health Communities Model

04/14/2017 - The Center for Medicare and Medicaid Innovation's Accountable Health Communities models at the Centers for Medicare and Medicaid announced 32 participants to serve as local support for linking clinical and community services in a a five-year pilot program. The goal of the model is to address social needs, including food insecurity, to improve health outcomes and reduce healthcare costs.

The AHC models are one of many models being tested by CMMI that provide opportunities for RDNs to demonstrate their value, and the Academy continues to meet with CMMI to advocate for the inclusion of RDNs/nutrition services in models being tested.
